Credit Card on File- Please note that the credit card on file will not be charged until after the appointment. However, if the appointment is cancelled with less than 24 hours notice or if you fail to show up, the card will be charged the full amount of the missed appointment. If you would like to use a HSA card for treatment, you may put it on file. While consultations are complimentary, please note that your card will be charged $15 if you fail to show up for the consultation or cancel with less than 24 hours notice.
Medicare- I am unable to accept Medicare patients at this time but would be happy to refer you to a local chiropractor who accepts Medicare.
Directions- When entering the address into GPS, please leave out the suite number.
COVID Policies- Please look for a mandatory COVID-19 symptom survey in your email inbox 4 hours before your scheduled appointment. This survey must be completed prior to entering the building before every appointment. When you arrive in the parking lot, please text (317) 606-9466, and I will text back as soon as I’m ready for you. A mask is required inside the building.
Intake Form for New Patients- Once your first appointment (New Chiropractic or Acupuncture Patient) is booked, you’ll receive a confirmation by email. In this email, you will find a link to the intake form. This intake form must be filled out completely prior to your first appointment. This intake form does not have to be completed prior to appointments for consultation only.
